£42??
Christ on a fucking trike, what do I pay National Insurance for? My "free at the point of delivery" NHS dentist wants to charge me £42 for a cunting filling.
I do already. It would be nice of the government to stop threatening to kidnap me because I no longer desire to use or pay for their extremely poor service.
I would recommend that people have a quick go at a private dentist just to see how much better a private GP service would be over the current extortion funded GP disaster.
Remember private patients are customers, NHS patients are costs.
I don't think it does cost less than that to go private, though. Probably guaranteed a better service. Mine is kicking his heels at the moment having referred me to Dr Diamond Drill and his magic cameras for my thrilling drilling experience.
My wife recently had a broken tooth replaced with a crown here in Canada. It cost £800 as we don't yet have a "dental plan" However, the treatment was first class. The dentist remarked that my wife had "English teeth" something he sees often as for some inexplicable reason lots of people are emigrating from Britain. He remarked that the dentists in the UK do the bare minimum to salvage teeth. Sadly I have to concur. I wouldn't mind so much but for the creeping privatisation. Year on year the costs go up and we still keep getting hammered for NI contributions, but the treatment doesn't improve.

Well, if your photograph is not too heavily re-touched, I'd want a lot more than £42 to get anywhere close to your mouth.

Go private, send your Health Authority the bill. If they don't pay, take them to Small Claims.
Or just don't go at all. Most dentists create problems rather than fix them.
